2019年2月14日 · We determined a threshold optic nerve area on MR imaging that predicts a clinical diagnosis of optic nerve atrophy and assessed the relationship between optic nerve area and retinal nerve fiber layer thickness measured by optical coherence tomography, an ancillary test used to evaluate optic nerve disorders.

Idiopathic optic neuritis is evaluated by MRI, but the findings are non-specific. In the acute phase, the optic nerve is enlarged with high-intensity signal on T2-weighted or STIR images and has a contrast effect on post-contrast-enhanced T1-weighted image (Figure 1).

Imaging at 3 T MRI scanners offers superior SNR as well as spacial resolution to evaluate orbital structures as well as intracranial pathologies, as compared with 1.5 T scanners. Thin-section high-resolution spin-echo T2-weighted images at 3 T depict the optic nerves and orbital anatomy much better than 1.5 T scanner. [6]

The best imaging modality to investigate optic neuropathy etiologies is an orbital MRI. The most common diagnoses overall were compressive optic neuropathy (25%), optic neuritis (22%), multifactorial (16%), and glaucoma (13%); retinopathy was only seen in 2.4% of cases.

Unlike other clinical MRI sequences for the optic nerve, diffusion MRI provides a quantitative, objective marker of optic nerve pathology and seems sensitive to subtle microscopic tissue changes, such as early selective axonal loss, that are not easily recognized by radiologists, referring physicians and sometimes patients.
